Abstract

Introduction: Polytrauma, a severe and emergent clinical syndrome, arises as a consequence of high-energy external force generating multiple injuries to more than one organ system. Such injuries are not only life-threatening but also accompanied by severe physiometabolic derangements. Polytrauma is responsible for over 20,000 deaths annually in high-income countries such as Germany. It is the leading cause of death and disability in youth and children and is accountable for 80% of deaths among adolescents and 60% of child mortality, primarily between the 25–34-year age group. Socioeconomic burdens are considerable.

Purpose: The presentation aims to give an overview of contemporary knowledge and management of polytrauma with emphasis on early intervention and survival.

Key themes of the 'lethal triad' ('lethal triad' concepts of hypothermia, coagulopathy, acidosis), systematic trauma survey (primary and secondary survey), and ATLS-based 'scoop and run' model are covered extensively. Time-critical concepts, such as the 'Golden Hour' and 'Platinum 10 Minutes,' are analyzed in the context of early intervention and survival. We also address the pivotal role of trauma systems and team organization in the successful management of polytrauma. Whether vertical versus horizontal integration models apply to delivering trauma care is contentious. Additionally, whether trauma scoring systems—namely, Injury Severity Score (ISS), Revised Trauma Score (RTS), and Trauma and Injury Severity Score (TRISS)—are applicable and clinically useful as triage, prognosis, and allocation tools is debated.

Conclusion: Effective management of polytrauma is a multidisciplinary process, and your role is valuable. It requires systems-based, multidisciplinary management that begins in the prehospital setting and extends to definitive surgical and critical care. The application of trauma scores and systematic protocols (ABCDE-FGH) enhances decision-making and outcomes. Institutional familiarity and adherence to standardized pathways of trauma care are essential to maximize survival and minimize long-term disability.
