Abstract

Introduction: Albania faces a persistent, high burden of trauma, with media and police reports often reflecting figures comparable to a state of conflict. The trauma mortality rate is estimated at 13.7 per 100,000 population, highlighting a significant public health crisis, further underscored by a high frequency of traffic fatalities. Despite over two decades of political and structural changes since 1992, the question of effective and coordinated trauma management remains largely unanswered. While the state and the Ministry of Health, through entities like the National Center of Medical Emergency (NCME), bear the primary responsibility, the current organizational efficacy is under scrutiny.

Objective: This paper aims to analyze the systemic inadequacies within Albania’s trauma care delivery, focusing on the disparity in emergency medical services (EMS) training, the organizational role of the NCME, and the critical need for adopting and standardizing internationally recognized trauma education protocols, such as Advanced Trauma Life Support (ATLS}

Discussion: The trauma system is currently evolving toward a centralized hub-and-spoke model, focusing resources at the University Hospital of Trauma. International evidence supports this shift; for example, the introduction of similar centralized systems in England has been associated with a notable 19% increase in six-year survival rates.

However, in Albania, while the NCME coordinates pre-hospital transport, substantial gaps remain. Although some medical staff have completed pre-hospital training (e.g., PhTLS®), the educational status of medical personnel outside the capital region remains inconsistent.

The challenge lies in scaling expertise: ATLS student courses have historically been implemented sporadically and individually. The Albanian Society for Trauma and Emergency Surgery (ASTES) has now acquired the organizational rights to formalize the implementation of ATLS protocols in the country.

Conclusions: Effective trauma management requires more than just administrative changes; it demands uniform, evidence-based training. The formal, widespread adoption and implementation of the ATLS course by ASTES is critical to standardize the initial assessment and resuscitation of injured patients, ultimately bridging the current gap in care quality and improving national survival outcomes.
